Group Life Insurance is available to eligible Employees. Contributions are made by payroll deduction and the University pays a percentage based on Employee salary. The amount of coverage is equal to two times the Employee salary, rounded up to the next highest thousand. Maximum benefit is $75,000. This benefit includes an accidental death and dismemberment benefit equal to the basic life insurance policy. The total cost is 30¢ per $1,000 of coverage. The University portion is shown in the chart listed in the Medical Insurance section labeled "University and Employee Contributions to Premiums." Note: 9, 10 and 11-month employees' annual premiums are paid over the academic year. Please see Group Life Summary Plan Description for plan details.

SUPPLEMENTAL LIFE INSURANCE

Benefits effective first of the month following the date of employment
Eligibility - regular employees working 30 hours per week (.75 FTE) or more
Enrollment form required, beneficiary form required
(Reference Policy 7.15)

Supplemental life insurance is available for eligible Employees and dependents. Premiums are paid 100% by the employee through payroll deduction. Premiums are based on age. Employees may elect up to $250,000 in increments of $10,000 with a $20,000 minimum. The first $50,000 requested is guaranteed without completing a statement of health. Spouse/Domestic Partners may elect up to $100,000 in increments of $10,000. The first $50,000 requested is guaranteed without completing a statement of health. Dependent children have two options for coverage:

  • Option 1 - $1,000 for 14 days to 6 months of age and $5,000 for 6 months to 25 years of age
  • Option 2 - $2,000 for 14 days to 6 months of age and $10,000 for 6 months to 25 years of age

Note: 9, 10 and 11-month employees' annual premiums are paid over the academic year. Late enrollment is available. Statement of Health and approval from the carrier is required for late enrollment. Please see Metlife brochure and the Metlife Policy Booklet for plan details.
