Vision Service Plan (VSP)
Benefits effective first of the pay period after 30 days of employment
Eligibility - regular employees working 20 hours per week (.5 FTE) or more
Enrollment form required

Vision services are available through the Vision Service Plan. Premiums are paid by the employee 100%. Late enrollment is available with a 90 day waiting period from the date Benefit Services receives the enrollment form. Benefits will be effective the first of the month following the 90 day waiting period. Benefits include $15 office visits, up to $120 towards frames or contacts, and $25 lenses. Some restrictions apply. Please see the Vision Summary for details.
